How to Claim Sweepstakes Prize?

Requirements and Guidelines
  1. Simply bring your two (2) valid IDs and the Winning ticket/s to the PCSO Main Office 2nd Floor
  2. Present your winning ticket/s at the Prize Fund Division, Accounting and Budget Department. Affix your signature at the back of winning ticket/s to establish your absolute ownership. Fill up claim form
  3. Proceed to Treasury Department. Present your Authenticated winning ticket/s and approved claim form for immediate preparation of your check
  4. Have your check signed by the Chairman / General Manager. Then properly receive your winning prize / check at the Treasury Department
  5. Encash your check at the Philippine National Bank / Land Bank of the Philippines located at Ground Floor
Conditions for Payment of Prizes
  • Prizes shall be paid to the holder of share/s of a winning ticket upon presentation and surrender of the same, provided that the ticket/s is/are free from any mutilation, erasure or stain, making any number/s therein illegible, and provided further that no court injunction restraining the PCSO to pay is received before payment
  • Prizes above 10,000.00 are subject to 20% tax pursuant to TRAIN Law

How to Claim Lotto Prize?

  • Write your name and affix your signature at the back of your winning tickets before claiming your prize
  • Prize amounting to 20.00 up to 10,000.00 can be claimed at any authorized Lotto outlet or at the PCSO Branch Offices in your area
  • Prizes from 10,001.00 and up to 300,000.00 can be claimed at the PCSO Branch Offices in your area or at the PCSO Main Office 2nd Floor
  • Prizes from 300,001.00 and up to JACKPOT PRIZE should be validated and claimed at the PCSO Main Office 2nd Floor

How to Apply as an Online Lottery Agent?

  1. SUBMISSION OF DULY ACCOMPLISHED LOTTO APPLICATION FORM TO PCSO:

    The application from shall attach the initial requirements:

    1. Sketch of the Location/Google Map of the proposed outlet site showing the exact name & no. of street. The proposed site must comply with the following distance requirements:
      1. Existing Lotto Outlet/s / (STL Outlets/Booths)
        1. Provinces - 100 meters
        2. National Capital Region (NCR) - 100 meters (minimum distance only)
        3. 200 meters from existing Draw Courts and Sub Stations
      2. School
        1. Elementary Schools - 100 meters
        2. Secondary Schools - 100 meters
    2. Picture of the proposed outlet site including its surrounding vicinity.
      • For National Capital Reoion (NCR) applicants within Metro Manila, submit accomplished application form at the Technical Evaluation & Monitoring Division located at the PCSO Main Office, 2F OR send via e-mail to [email protected] for National Capital Region, [email protected] for Southern Tagalog and Bicol Region (STBR), [email protected] for Northern and Central Luzon Department, [email protected] for Visayas Department, and [email protected] for Mindanao Department;
      • For applicants outside Metro Manila, submit your application form at the PCSO Branch Office covering your proposed outlet location.
  2. PAYMENT OF APPLICATION FEE

    Pay the application fee of 2,500.00 per proposed site to PCSO Main Offce Treasury Department/Branch Office covering your proposed site location.

  3. INITIAL INSPECTION AND EVALUATION OF THE PROPOSED SITE

    Your application will be initially inspected and evaluated. Once the proposed site passes the criteria, the applicant will be informed officially within 5 working days through the information provided in the application form.

  4. SCREENING OF APPLICANT'S QUALIFICATION AND SUBMISSION OF ADDITIONAL REQUIREMENTS

    Upon receipt of the notification from PCSO for the processing of the application for approval, applicant must submit the additional requirements within 5 days to the NCR department or branch office where the proposed site is located:

  5. SUBMISSION OF PICTURES OF INTERIOR AND EXTERIOR OF THE CONSTRUCTED LOTTO BOOTH (POST-APPROVAL REOUIREMENTS)

    Upon receipt of the Notice of Approval from PCSO, the applicant must accomplish the following within 15 days:

    1. submit pictures of the interior and exterior of the conslructed booth showing compliance with the booth requirements listed below;
    2. pay the lnstallation fee of 10,000.
    Booth requirements:
    • Floor space is at least 2m x 2m. Business counters measuring at least 36"x24".
    • Dry floor space with proper ventilalion.
    • With at least one (1) wireless, fixed landline telephone OR cellphone for hotline operation use.
    • For security reasons, all Lotto outlets should be enclosed and must be facing the direction of human traffic/entry. Frontage must face the main hallway/corridor or to the direction of major traffic flow.
    • Outlets located within a commercial building or complexes, with 24-hour security, Lotto booths need not be enclosed.
  6. POSTING OF CASH BOND OR FILING OF SURETY BOND TO GSIS

    File Surety Bond to GSIS or post Cash Bond:

    *Note that, pursuant to Section 1 of Rule VIII of the Revised Rules and Regulations of Lotto Operation, PCSO shall have the sole right to determine which form of bond shall be required to the applicant.

    Cash Bond:
    • NCR - 300,000.00
    • Cities Outside NCR - 200,000.00
    • Municipalities Outside NCR - 100,000.00

    (700,000.00 coverage)
    • 13,864.32(GSIS) (Premium br 2-year period)
  7. SIGNING OF AGENCY AGREEMENT

    Approved applicant will sign the Agency Agreement at the NCR Department or concerned Branch Office.

  8. TRAINING, AND SECURING OF SUPPLIES

    Approved applicant and his/her teller must undergo 1 day training and may secure lotto supplies at the PCSO Warehouse for NCR or Branch Office.

On-Line Lottery Agents’ Application Requirements?

  1. APPLICATION REQUIREMENTS:
    FOR CORPORATION / PARTNERSHIP APPLICANT
    QUALIFICATION OF APPLICANT:
    • In case of Partnership Applicant:
      1. Partners must be Filipino Citizen; AND
      2. Partnership Applicant must be duly registered and existing under Philippine Laws.
    • In case of Corporation Applicant:
      1. Must be fully owned and controlled by Filipinos;
      2. Corporate Officers must all be Filipinos; AND
      3. Corporation Applicant must be duly registered and existing under Philippine laws.
    ADDITIONAL REQUIREMENTS (UPON SITE APPROVAL):
    • Bio-data of its Board of Directors and Ofricers, or Partners;
    • NBI Clearance of its Board of Directors and Officers, or Partners;
    • Secretary's Certifcate authorizing the applicant to represent the Corporation / Partnership;
    • Two (2) latest passport size lD picture with white background of the authorized representative;
    • Proof of ldentification (two (2) valid lDs) of its Board of Directors and Officers, or Partners;
    • Income Tax Return (ITR) for the immediate preceding year;
    • Latest Audited Financial Statement;
    • Certified True Copy of SEC Registration, Articles of lncorporation (for corporation applicant), Articles of Partnership (for partnership applicant), By-Laws, and latest General lnformation Sheet (GlS);
    • Company TIN; and
    • ln case the proposed site is:
      1. Residential building and owned by applicant or his/her immediate family:
        Barangay Certification showing thatthe applicant resides in the proposed sile.
      2. Commercial and owned by applicant or his/her immediate family:
        Business permit issued by the city or provincial government.
      3. Leased propetty:
        1. Duly notarized Letter of Agreement between the owner of the establishment and applicant showing that the proposed site will be leased to the applicant for the Lotto outlet; OR
        2. lf currently leased, duly notarized Contract of Lease.
    FOR INDIVIDUAL APPLICANT
    QUALIFICATION OF APPLICANT:
    • Filipino Citizen and at least 18 years old
    ADDITIONAL REQUIREMENTS (UPON SITE APPROVAL):
    • Any two (2) valid identifcation cards with at least one (1) Government issued lD;
      • National lD
      • Postal lD
      • Driver's License
      • Passport (valid)
      • UMID ID
      • Tax ldentification Number (TlN)
      • NBI Clearance (valid)
    • Two (2) latest passport size ID picture with white background of applicant
    • ln case the proposed site is:
      1. Residential building and owned by applicant or his/her immediate family:

  2. REQUIREMENTS FOR SURETY BOND POLICY (GSIS):
    • Duly accomplished and notarized Bond application form for Lotto or Agent;
    • Duly accomplished and notarized Co-signer's Form (Co-signer has to be employed or with business);
    • Two (2) valid government issued IDs of the Lotto Agent (photocopy with three (3) specimen signatures);
    • Two (2) valid government issued IDs of the Co-signer (photocopy with three (3) specimen signatures);
    • Copy of Agency Agreement; and
    • Agency Agreement Certification issued by PCSO (because Agency Agreement is not yet fully executed).

What are the Requirements for Medical Access Program (MAP)?

  • Confinement
    • Original/certified photocopy of medical abstract with license number and signature of the doctor
    • Official Statement of Accounts with signature of the Billing Officer less PhilHealth and other deductions
    • Valid ID of the patient (photocopy)
  • Erythropoietin (dialysis injection)
    • Original/certified photocopy of medical abstract with license number and signature of the doctor
    • Original prescription of epo injection with license number and signature of the doctor
    • Original endorsement with official quotation (applicable for Dialysis Center and hospital)
    • Valid ID of the patient (photocopy)
  • Cancer Treatment
    • Chemo drugs
      • Original/certified photocopy of medical abstract with license number and signature of the doctor
      • Original prescription with license number and signature of the doctor
      • Original or Certified True Copy of the Treatment Protocol
      • Three (3) official quotation from different supplier if not available from the hospital/dialysis center
      • Valid ID of the patient (photocopy)
    • Radiation therapy
      • Original/certified photocopy of medical abstract with license number and signature of the doctor
      • Official quotation from the hospital
      • Certification of PhilHealth benefits availment from the hospital
      • Valid ID of the patient (photocopy)
  • Specialty Medicines (Hemophilia, Post Kidney transplant medicines, Rheumatoid Arthritis, Anti-Lupus, Immunocompromised Disease (IVIg), Psoriasis, Orphan Diseases, Idiopathic Thrombocytopenic Purpura (ITP), Thalassemia and Neuro-psychiatric)
    • Original/certified photocopy of medical abstract with license number and signature of the doctor
    • Original prescription with license number and signature of the doctor
    • Three (3) official quotation from different supplier if not available from the hospital
    • Valid ID of the patient (photocopy)
  • Hemodialysis
    • Original/certified photocopy of medical abstract with license number and signature of the doctor
    • Original endorsement with official quotation from the Dialysis Center and hospital
    • Certification of PhilHealth benefits availment from the Dialysis Center and hospital or Certification of PhilHealth exhaustion
    • Valid ID of the patient (photocopy)
  • Laboratory (blood chemistry) and Diagnostic and Imaging procedures
    • Original copy of medical certificate with license number and signature of the doctor
    • Letter of Acceptance for Guarantee Letter
    • Official quotation from the hospital/Diagnostic Center
    • Valid ID of the patient (photocopy)
  • Implant and Medical Devices (pacemaker, septal occluder, valves and PCSI devices such as Stent, Balloon catheter)
    • Original copy of medical certificate with license number and signature of the doctor
    • Letter of Acceptance for Guarantee Letter
    • Three (3) official quotation from different supplier
    • Valid ID of the patient (photocopy)

What are the Requirements to become a Medicines Retailers?

Interested medicines retailers need to submit the following documentary requirements:

  1. Letter of Intent addressed to the PCSO General Manager;
  2. Company Profile (historical background, location/address, vision and mission statement, goals, organizational and management structure, programs, services and coverage areas, facilities and equipment, and staff composition);
  3. Audited Financial Statements for the last 3 years;
  4. Securities and Exchange Commission (SEC) Registration Certificate, latest Articles of Incorporation, By-Laws and latest Information Sheet or Department of Trade and Industry (DTI) Registration Certificate;
  5. License to Operate as a Drug Retailer issued by Food and Drug Administration (FDA);
  6. Valid Business Permit;
  7. Picture of proposed or existing pharmacy or drug store and vicinity map;
  8. Formulary and Pricelist of cancer treatment and specialty medicines with 25% or higher discount to be validated against prevailing market price and Department of Health - Drug Price Reference Index (DOH-DPRI).
