DATA PRIVACY MAP Online Application

In the pursuit of its mandate under Republic Act (RA) No. 1169, as amended, and to better serve its indigent or financially challenged clientele needing financial assistance for medical/hospitalization assistance, the Philippine Charity Sweepstakes Office (PCSO or “We”) collects and processes personal information and sensitive personal information as defined in Section 3 of RA No. 10173 (The Data Privacy Act of 2012), (collectively referred here as personal data), including those sourced from its official website.

As a responsible data custodian, we commit to keep such personal data secure from any unlawful or unauthorized processing, in line with RA No. 10173, and the fundamental right to privacy of every individual.

This Privacy Notice defines how we manage the information we collect or generate via our official website and other linked online services.

INFORMATION WE COLLECT

We collect the following personal data primarily via the linked web applications found in the PCSO website (www.pcso.gov.ph) under “E-SERVICES”:

  1. Full name;
  2. Age;
  3. Sex;
  4. Date and place of birth;
  5. Civil status;
  6. Home address;
  7. Contact numbers;
  8. Email address
  9. Medical documents (Medical Abstract, Prescription, hospital billing statement, diagnostic and curative, and therapeutic procedures needed, hospital billing statements, etc.);
  10. Socio-economic profile of applicant or beneficiary (i.e., employment status/financial capacity, gainfully employed family members, if any, and related documents); and
  11. Government issued Identification Card (ID), which may include face/photo or handwriting and signature.

We also use internet or web cookies to help us maintain and improve the website to enhance user experience. Thus, when you visit our website, the following information are also recorded via Google Analytics:

  1. Browser type and version;
  2. Browser plug-in types and versions;
  3. Date and time of connection;
  4. Length of visits to certain pages;
  5. IP address; and
  6. Page interaction information (e.g. scrolling, clicks and mouse-overs).

Moreover, data gathered and processed by Google Analytics are used to generate a report on the following:

  1. Most viewed pages (per day and per month);
  2. Number of regular users and new users;
  3. Landing pages;
  4. Social networking site which has the most referrals;
  5. Location of most users; and
  6. Most common devices and browsers used to access the website.

PURPOSE OF COLLECTION

PCSO collects data to evaluate and process applications for the following

  1. Confinement;
  2. Chemotherapy;
  3. Dialysis Treatment;
    1. Erythropoietin Injection (EPO); and
    2. Hemo / Peritoneal Treatment;
  4. Medicines for Hemophilia; and
  5. Medicines for post-transplant procedure.

The personal data collected will only be processed and used by PCSO and will not be disclosed to any third party or organizations outside PCSO unless otherwise stated, or required by law, or with prior consent.

THIRD-PARTY DISCLOSURE/DATA SHARING

We treat the personal data confidential and disclose them to others only if necessary for compliance with, with a legal obligation, or where allowed or required by law.

RETENTION OF INFORMATION

Except when otherwise required or permitted by law, collected personal data are kept/retained only as long as they are necessary to achieve the purposes declared herein. After which, they are deleted or disposed of in a safe and secure manner.

RIGHTS OF DATA SUBJECTS

Individuals (data subjects) whose personal data we collect may invoke their rights as provided for under Section 16 of RA 10173 at any time, in accordance with the provisions of the law. On the other hand, if there are inaccuracies in their records that need to be corrected or updated, data subjects shall be required to immediately notify PCSO thru the Charity Assistance Department (CAD) or any of its Branch Offices nationwide; provided, that PCSO shall be free and harmless from any inaccurate, malicious or fictitious information from the data sharer without prejudice to the filing of appropriate administrative, civil and criminal action and other legal relief under existing laws for fraudulent claims, if any.

SERVICE/SOLUTION PROVIDERS

The sharing is covered by data sharing agreement or non-disclosure agreement and is implemented through secured means.

CHANGES TO THIS NOTICE

This Privacy Notice was issued on April 21, 2020 to facilitate the availability and online access to PCSO’s Medical Assistance Program (MAP) and will remain effective unless there is a revision, which will be effective immediately after its posting on the PCSO website. PCSO will make every effort to announce to the public any changes made on this Notice.
